Pro tip: That ASME code class in Cleveland made me question the standard practice for pre-heating 1.25 chrome
The instructor showed us a failed weld from a refinery in Texas where skipping the pre-heat on a 2-inch thick joint seemed fine until the hydro test, so now I ask for the PQR before we even set up the rig, but does anyone else think the book rules are sometimes too rigid for field repairs?

Nah, the book rules are there for a reason... seen too many guys get burned, literally and on paper, trying to cut that corner. That PQR is your only real proof it was done right.

Saw a guy fail an audit last year over a missing PQR stamp. Paperwork is the only thing that matters when the inspector shows up.
