Check your mix design, I found our slump test was lying to us

Was on a slab pour last week near Austin, looked fine on paper. The spec said 4 inch slump but the mix kept slumping to 6 and we kept adding water on site. I finally dug into the batch ticket and the plant used a different aggregate source than the design called for. That changed the water demand by almost 8 gallons per yard, nobody caught it for 3 days. We had to core test two sections and one failed at 3,200 psi instead of 4,000. Has anyone else had a plant swap aggregates without telling you? How do you spot it before you pour?
