Why does nobody flag reused passwords until you get burned once

I used the same password for my bank, email, and a random gaming forum for like 6 years. Then last February I got a text about a login from a new device in Tucson, where I have never been. Turns out the forum had a breach and someone scripted my password across other sites. That one lazy habit almost drained my checking account, and I only caught it because the bank's alert went to spam. Has anyone else had that blind spot, or do you all actually rotate passwords like the guides say?
