My main client's payment system went down for 48 hours and I almost missed rent

That "systems failure fee" name is clever, makes it sound less personal. But is chasing a wire really that big of a deal? A few emails over a couple days. Ray_miller41 has a point about making them fix the portal, but a whole fee for that feels like nickel and diming unless it's a constant problem with the same client.

That rule about using a page or quote is so smart. It stops people from just saying "well, that's how I feel." Like, if someone says a character is selfish, you have to find the line where they took the last piece of pie while someone else was crying. It turns a messy fight into a real search for proof. Makes you actually see the book again instead of just your own version of it.

Just lost $75 to a fake wire fee scam

Yeah, they're getting really good at making those look real. I got one last month that even had a fake customer service number that picked up with a recording. It's scary how much effort they put into this stuff. Always check the actual sender email address, not just the display name. That's usually the giveaway, but you have to look close. Sorry that happened to you, it's an expensive lesson.

I keep seeing people skip the title block on shop drawings

Blank title blocks are the reason my filing system is a total mess. @jordanc32 has the right idea with the blank check revenge. My own paperwork is a safety hazard.
